The following CRTASA member application forms are intended for all new applicants seeking CRTASA membership registry that either have not previously applied for membership or who may be existing CRTASA members that have acquired a new service animal, a new service animal-in-training, or a new therapy animal that has not previously been registered with CRTASA. CRTASA "Renewal" Membership Application Forms

The following renewal CRTASA member application forms are intended for all previously registered CRTASA members that are seeking re-registry for their previously CRTASA registered service animal, service animal-in-training, or therapy animal. Simply click on the provided hyperlink below to download and complete suitable application as per identified instructions on the form. For all newly acquired working animals the "New" Member Application forms must be used!

CRTASA Photo ID Card Replacement Forms

The following CRTASA Photo ID Card Replacement Forms are to be used by existing valid CRTASA members who need to have their card reissued by CRTASA as a result of their originally issued card becoming damaged, lost, or stolen prior to their annual CRTASA membership term expiring.
