CRTASA Membership Policy
New CRTASA Applicants
New CRTASA applicants with an official working animal interested in becoming a registered CRTASA member are required to provide CRTASA with all documentation specified on the CRTASA New Membership Application Form. Application requires submission of a recent headshot "Passport-Type" colour photo of the applicant and a separate colour headshot of the applicant's working animal being registered.
Full payment for CRTASA's annual membership fee must also accompany each completed CRTASA application. Accepted forms of payment are limited to either certified cheque or money order. All payments are considered non-refundable due to the time and resources involved in verifying and processing an applicant's CRTASA membership application.
CRTASA membership and CRTASA Photo ID Card are deemed valid for a one (1) year term with the provision that the CRTASA member's and/or their working animal status remain unchanged during that one (1) year period. CRTASA membership and CRTASA Photo ID Card must be renewed on an annual basis with CRTASA, at least one month prior to the expiry date noted on the CRTASA Photo ID Card or immediately when the CRTASA member's status changes and/or a new service animal, service animal in training or therapy animal is acquired for which a CRTASA Photo ID Card has not been requested or previously issued.
Each CRTASA membership applicant and/or a power of attorney applying on behalf of a CRTASA membership applicant with a disability further acknowledge and provide complete implied consent to CRTASA to contact any/all third parties listed on the applicant's completed CRTASA application form including any treating doctor(s) needing to verify the use of a service animal to assist the applicant with daily activities based on a disability, and/or any other third parties that provided supporting confidential and private documentation about the applicant, and/or the official status of the applicant's service animal, service animal in training or therapy animal. Third party contact and discussions of applicant's personal and confidential information are considered for verification purposes only to help verify and process where feasible the applicant's CRTASA membership and issuance of a valid CRTASA Photo ID Card for the applicant and their official working animal according to CRTASA's membership program qualification criteria.
CRTASA retains the sole discretionary right to issue CRTASA membership and a valid CRTASA Photo ID Card to applicants and their working animal that are able to meet and satisfy the terms and conditions of CRTASA registry service program and membership criteria as identified in the CRTASA registry program policy. Furthermore, CRTASA retains the sole discretionary right to deny, refuse or revoke its CRTASA membership services and CRTASA Photo ID Card at any time to anyone. All CRTASA members further agree to immediately surrender to CRTASA upon CRTASA's request their CRTASA Photo ID Card that expires and/or becomes invalidated and/or revoked by CRTASA due to any breach or contravention that CRTASA deems to believe has occurred or been committed against its CRTASA registry service and/or membership program policy.
Continuation or new use of any expired and/or invalidated and/or revoked and/or illegitimately received CRTASA Photo ID Card by any party(s) shall be deemed as misrepresentative and fraudulent use of CRTASA registry services and/or membership benefits otherwise exclusively awarded and held by valid CRTASA members. Any person found to falsify or to use an expired and/or invalid and/or revoked and/or illegally assigned CRTASA Photo ID Card for themselves and/or by a third party with an animal not recognized as an official service animal under CRTASA's membership qualifying criteria, provides CRTASA with the sole and exclusive right to pursue any/all necessary legal recourse available against any and all direct offender(s) and/or third party violator(s) including by legal action(s), torts, claims, liability, loss, and expense(s) (including damage awards, settlement amounts, and reasonable legal fees), against any / every offending party for the purpose of protecting CRTASA's integrity and promoting its national registry service program designed to promote the working status of service animals, service animals in training and therapy animals and their rightful owners.
